Overview
We understand managing your health is crucial, and our annual review process ensures you receive comprehensive care tailored to your needs. This guide will walk you through the steps of the review process, which is based around your month of birth for convenience and consistency.
Long Term Condition/Medication Review Process Steps
Invitations/Reminders
Each year, during the month prior to your month of your birth, you will be invited to participate in an annual review.
Blood Tests and Blood Pressure Checks with the Healthcare Assistant (HCA)
Your first appointment may be with our Healthcare Assistant (HCA), who will conduct essential preliminary tests:
– Blood Tests: These help us monitor various aspects of your health, such as blood sugar levels, cholesterol, and kidney function.
– Blood Pressure Checks: Regular monitoring of your blood pressure is vital to detect any changes that may require adjustments to your treatment.
Health Checks with the Practice Nurse
Following your initial tests, you will meet with our practice nurse for a thorough health check (you may need several appointments with the nurse, depending on the number of health conditions you live with):
– Physical Examination: The nurse will assess your overall physical health, looking for any signs or symptoms that need attention.
– Lifestyle Review: We will discuss your diet, exercise, smoking, and alcohol consumption habits to provide personalized advice.
Structured Medication Review with the Pharmacist
A major part of your annual review is a detailed and comprehensive structured medication review with our pharmacist:
– Medication Assessment: The pharmacist will review all your current medications, including their effectiveness and any side effects.
– Drug Interactions: We will check for potential interactions between your medications and any new prescriptions or over-the-counter drugs.
– Optimizing Treatment: If necessary, the pharmacist will suggest changes to your medication regimen to improve your health outcomes.
Why This Review is Important
Regular reviews are essential for managing long-term conditions and ensuring your treatment remains safe and effective.
This process allows us to:
– Detect and address potential health issues early.
– Adjust treatments based on your current health status.
– Provide support and advice to help you manage your condition effectively.
